• Insurance claim submission assistance

Services may require documentation (e.g., prescriptions, diagnosis codes, insurance information).

3. Eligibility

 

 

To use services or purchase products, you must:

• Provide accurate and truthful information

A. Fees

 

 

Consultation fees may apply. Fees are non-refundable unless otherwise stated.

 

 

B. Rescheduling & Cancellations

 

 

We may allow rescheduling when available. No-shows or last-minute cancellations may result in forfeited consultation fees.

 

 

C. Insurance Guidance

 

 

We assist with insurance verification and claim submissions, but coverage is not guaranteed.

Final determination belongs to your insurance provider.

 

 

 

 

5. Payments & Refunds

 

 

 

A. Accepted Payment Methods

 

 

We accept major credit/debit cards, HSA/FSA when applicable, and payment plan services such as Afterpay or Klarna (when available).

 

 

B. Refunds

 

 

Due to the nature of cranial prostheses and health-related items, all wig sales are final once they leave our possession, for hygiene and medical compliance reasons.

 

Returns or exchanges may be considered only if the product is defective upon arrival.

 

 

 

 

6. Insurance & Billing Disclaimer

 

 

Wig RX LLC is not responsible for denied claims, changes in insurance coverage, or incomplete documentation from providers.

 

You understand and agree:

 

• Insurance coverage varies

• Preauthorization does not guarantee payment

• You may be responsible for out-of-pocket costs if your insurer denies payment

 

 

We do not fabricate prostheses; we supply and fit medical wigs.

 

 

 

 

7. User Responsibilities

 

 

You agree to:

 

• Provide accurate medical and insurance information

• Submit required documents (prescriptions, diagnosis codes, etc.)

• Use products according to recommended guidelines

• Not misuse or falsify health information

 

 

 

 

 

8. Intellectual Property

 

 

All Wig RX content—including logos, text, images, videos, designs, and educational materials—is owned by Wig RX LLC and may not be copied, distributed, or used without written permission.

 

 

 

 

9. Website Use & Restrictions

 

 

You may not:

 

• Access or use the site for fraudulent or illegal purposes

• Attempt to interfere with the website’s security

• Disrupt services or reverse engineer site software

• Upload harmful materials

 

 

Violations may result in termination of access.

 

 

 

 

10. Limitation of Liability

• Allergic reactions to products

• Improper use or maintenance of wigs

• Damages from third-party delays or shipping carriers

• Denied insurance claims

• Medical or emotional outcomes related to hair loss

 

 

Our services do not replace medical advice from a licensed healthcare provider.

 

 

 

 

11. Privacy & HIPAA Compliance

 

 

Your privacy is protected under our Notice of Privacy Practices (NPP) and HIPAA standards.

 

By submitting information, you consent to our handling of your PHI as described in our NPP.

 

 

 

 

12. Changes to Terms

 

 

We may update these Terms occasionally. Changes take effect immediately when posted on our website.

 

 

13. Insurance & Reimbursement Terms:

Insurance Coverage Disclaimer

Wig RX LLC provides assistance with insurance coordination and documentation for cranial prostheses. However, coverage is determined solely by your insurance provider. We do not guarantee approval, reimbursement, or payment by any insurer.

 

Client Responsibility

It is the client’s responsibility to:

 

• Verify benefits and coverage with their insurance carrier prior to purchase.

• Understand plan-specific requirements, including deductibles, co-pays, pre-authorization, and coverage limits.

• Submit all required documentation to their insurer in a timely manner, unless otherwise agreed in writing.

 

 

Documentation Support

Upon request, Wig RX may provide supporting documentation such as invoices, product descriptions, HCPCS codes, and proof of medical necessity as required for claims submission. We do not control how insurers interpret or process this documentation.

 

Out-of-Network Status

Unless explicitly stated otherwise in writing, Wig RX operates as an out-of-network provider. Payment is due at the time of service. Any reimbursement issued by the insurance company will be paid directly to the client unless an assignment of benefits is approved.

 

No Guarantee of Reimbursement

Wig RX is not responsible for denied, delayed, reduced, or unpaid claims. Lack of reimbursement does not release the client from payment obligations.

 

Prescription & Medical Necessity

Clients seeking insurance reimbursement must provide a valid prescription and/or letter of medical necessity from a licensed healthcare provider when required by their insurer.

 

Claim Submission

Unless otherwise agreed, claim submission is the responsibility of the client. Wig RX may offer guidance but does not act as a billing agent or guarantee claim processing.

 

Changes in Coverage

Insurance policies and benefits may change at any time. Wig RX is not responsible for changes in coverage, eligibility, or plan terms that affect reimbursement.

 

Private Pay Option

Clients may choose to proceed as private pay regardless of insurance eligibility. All private pay purchases are subject to Wig RX’s standard payment and refund policies.

 

No Medical Advice

Wig RX does not provide medical advice or diagnoses. All medical determinations must be made by a licensed healthcare provider.
